A person possessing a weapon where prohibited will be dismissed from the training program, will forfeit tuition and may be subject to prosecution. I understand that weapons are strictly prohibited on Wings Health Care Training campus and on the property’s of the nursing care facilities for the clinical training portion of the course. I also understand that Wings is a training school and does not guarantee me a job. This program is training in preparation for certification testing and job readiness. I understand that the training program consists of 95 hours of classroom, lab and clinical training. If an applicant is found to have a felony conviction for certain specified crimes, they cannot be offered employment. I also understand that each Medical Facility and Health Agency in the State of Oklahoma is required to do criminal background check through the Oklahoma State Bureau of Investigation. Students must have sufficient physical capabilities to complete the training program and avail themselves of employment opportunities. Pre-Requisite for Enrollment: Students must have good hearing and eyesight with corrective appliances as needed. If the x-ray result is positive, the student will be dismissed from the course with a refund of $200 minus $10 a day for each class day attended.

If the result of that skin test is positive, the student will be sent to the Department of Health to receive a chest x-ray. If a student fails to pass the drug test, they will automatically be withdrawn from the class and will not receive a refund.Īdmission Requirement: Each student is given a TB skin test on the first day of class. And, that this agreement requires a student to consent to a drug test if it is requested by Wings Health Care Training during the course of the program. I understand that as a condition of admission to this program, I will be required to sign a Drug Testing Waiver Agreement. Once the OSBI has been requested, the fee is not refundable. The student will then receive a refund of $525.00. If a student does not satisfactorily pass the background check, they will be automatically withdrawn from the class. Pre-Requisite for Enrollment: An Oklahoma State Bureau of Investigation background check will be done on each student. ONCE A STUDENT HAS ATTENDED CLASS, NO REFUND IS AVAILABLE. 3.) Student must have contacted the business office before 7pm the Friday prior to the beginning of class. 2.) Student must have paid their tuition in full. Refund Policy: To be eligible for a $525 refund, a student must meet ALL three criteria: 1.) Student must not have attended any class. Upon the second incident, I understand that I will be asked to leave the training program and no refund will be issued to me.
It is my understanding that if I do leave their supervision, I will be warned once in writing. Any incidents, should they occur, should be reported immediately. I also understand that this rule is for the safety of myself, fellow students, my instructor and the residents of the nursing care facility. I understand that under no circumstances am I to leave their supervision and perform any other duties. I understand that during my clinical training I will be under the supervision of a Clinical Trainer. Wings Health Care Training is licensed by the State of Oklahoma Department of Health and follows their guidelines for training for the Certified Nurse Aide/Home Health Aide Program for Oklahoma. I have read and understand the above training requirements and believe I am physically capable of performing them without injuring myself and others. Following is a list of functions performed during training. I understand that If I am pregnancy I must obtain a medical release from the Wings office to be signed by my Physician before beginning my training. Many of these duties require physical capabilities including being able to lift 75 lbs. The curriculum for this program is based on the duties required for job performance. Students under the age of 18 must have a high school diploma, GED certification or sign the enrollment agreement jointly with a parent or guardian. Pre-Requisite for Enrollment: Students that are 18 years of age or older are accepted. $25 Oklahoma State Bureau of Investigation Background Check: Pre-Enrollment Requirement Tuition Includes: $250 Student Kit: TB Skin Test, Training Certificate, and Use of Durable and Disposable Materials and Equipment needed for Training. $325 95 Hours of Class and Clinical Training .
